ERF FORMS

Most ERF forms are available online for your convenience. To submit a form, locate the form you need and click on the form title. Your form will open in a new window. The required fields will be highlighted for you to type in directly.

Once you have completed the form, print it out, sign it (unless it is stated on the form that a notary is required) and either email, mail or fax your form to the ERF office or upload your completed form to your myERF account. Your form(s) may be revised any time you deem necessary. Please contact an ERF Benefit Counselor regarding detailed instructions for submitting a new or revised Death Benefit Form.

There are three ERF forms that need to be notarized; Retirement Application, Refund Application and Beneficiary Application.
